AUTHORIZATION AND RELEASE

In return for being allowed to participate in volunteer activities and all related activities with and for Simply Hygiene, Inc., including any activities incidental to such participation (“Volunteer Activities”), the undersigned Volunteer or Parent/Legal Guardian of Volunteer if Volunteer is under age 18 (“Volunteer”) releases and agrees not to sue Simply Hygiene, Inc. or its agents, affiliates, licensees, successors and assigns, or its officers, directors, employees, sub-contractors and sponsors (collectively, “Simply Hygiene”) from all present and future claims that may be made by Volunteer, Volunteer’s family, estate, heirs or assigns for property damage, personal injury (including any first aid, treatment, or service rendered in connection with the Volunteer Activities), or death arising as a result of Volunteer’s participation in the Volunteer Activities wherever, whenever, or however the same may occur.
Volunteer understands that participation in the Volunteer Activities involves certain risks that are inherent in working in an industrial warehouse setting, including but not limited to exposure to chemicals and injuries from lifting, packaging, moving product and using equipment. Volunteer is voluntarily participating in the Volunteer Activities with knowledge of the risks involved, and Volunteer agrees to accept all risks of participation.
Volunteer hereby irrevocably consents and authorizes Simply Hygiene to use, publish, reproduce, create derivative works of, distribute, and publicly display Volunteer’s name, picture, portrait, likeness, and image (“Personal Information”) in any form, media, or content for any type of advertising, marketing or promotional purposes, including, without limitation, use on SIMPLY HYGIENE’s website and social-media pages.
Volunteer agrees and consents to Simply Hygiene’s irrevocable, worldwide title, interest, and intellectual property rights in and to any photographs, images (including electronic images), videos, audio, or other recordings of Volunteer or Volunteer’s Personal Information taken or created by or on behalf of SIMPLY HYGIENE and any derivative works made thereafter. Further, Volunteer hereby releases Simply Hygiene and waives any claims, demands, damages, losses, liabilities, and causes of action arising from the conducting of any of the above authorized uses of Volunteer’s Personal Information by Simply Hygiene, including, without limitation, any claims for copyright, moral rights, defamation, libel, invasion of privacy, publicity, personality, or exploitation of Volunteer’s name or likeness.
No representations have been made by Simply Hygiene or any other person to induce Volunteer to execute this Volunteer Waiver and Release. Volunteer warrants that before executing this release, Volunteer has read the same and understands it. Volunteer also expressly warrants that all understandings between Volunteer and Simply Hygiene are set forth herein and nothing contained herein shall obligate Simply Hygiene or any other party to pay any compensation to Volunteer for the Volunteer Activities or for its use of Volunteer’s Personal Information.
Volunteer understands that Simply Hygiene’s insurance may or may not cover accidents involving Volunteers participating in Volunteer Activities. In the event that Simply Hygiene’s insurance provides coverage for such Volunteer Activities, the Simply Hygiene policy pays only after the Volunteer’s insurance pays. Volunteer is expected and encouraged to obtain his or her own medical or health insurance coverage.
Volunteer understands that this document is intended to be as broad and inclusive as permitted by the laws of the State of Kansas and agrees that if any portion of this Agreement is invalidated, the remainder will continue in full legal force and effect. This Volunteer Waiver and Release shall be governed by and interpreted in accordance with the laws of the State of Kansas. Volunteer understands that this document is a contract which grants certain rights to and eliminates the liability of Simply Hygiene.
Volunteer certifies that he or she is at least 18 years of age or consent by a parent or legal guardian of the Volunteer has been obtained, as evidenced by the signature of such party below.

I am of legal age and am freely signing this agreement. I have read this Volunteer Waiver and Release and understand that by signing this form, I am giving up legal rights and remedies.
If under 18 years of age, consent by parent or legal guardian is required below. If no parent or legal guardian signs below, Volunteer is representing to Simply Hygiene that Volunteer is at least 18 years of age by signing above.
